Working from first principles:
If it does not include a serious injury and is outside the time frame then
it is no longer a RIDDOR report.
If, on reflection, you think it is something that the HSE needs to know
about then I would suggest that the organisation should inform them and
anticipate slapped wrists - or worse if it is something that was of such
severity that a reasonable employer should have known about. From the
details given regarding the particular case in question - I am not sure if
it is something that should be raised with the HSE - but the details are
very brief. A phone call to them for clarification may be helpful if you are
unsure.
