Physiological testing, once the domain of endurance athletes, can be a
useful tool in battling cardiometabolic diseases

BOULDER The treadmill is rolling along at a grand total of 4 mph, and I'm
chugging along at a shuffle, breathing into a mask. My exercise
physiologist, Kevin Nicol, pricks my fingertip for the first of five tiny
blood samples.

The blood samples tell him what's fueling my jogging: fat or
carbohydrates. At a shuffle, I'm burning fat. Later, running harder, I'll
burn only carbs.

Initially, the test was easy. It got tougher, of course, as Nicol and Igo
San Mill, director of sports performance at the new University of Colorado
Sports Medicine and Performance Center, where I took this test, sped up
the treadmill. But it's OK, the short-lived suffering at the very end of
my physiological testing was for science.

Elite endurance athletes come to the lab to get specific numbers they can
apply in a training regime that will improve their performance. Regular
folks like me, though, can come to the lab for a different takeaway: an
individualized prescription for exercise that can help fight off or even
reverse some chronic diseases.

San Mill has worked with professional cyclists and runners for years.
Elite endurance athletes form the only population out there that is free
of cardiometabolic diseases and issues, such as Type 2 diabetes and
metabolic disease, he said. So he's taking what he knows about the
metabolism of elite athletes and applying it to a much larger population:
people facing the health risks posed by inactivity.

When people decide they want to be more active or lose some weight, or
when a doctor tells them they need to exercise, people often don't know
what to do, San Mill said, and they often overdo it. "People go on this
drastic diet, they go to the gym and just kill themselves, ... and we know
that people drop out of these programs in six months," he said. "They fail
at losing weight. Many even get injured."

If those people had an individualized program, they wouldn't have to
suffer so much, and they'd get much better results in their overall health
at a cellular level, he said.
