If it's just for illustrative purpose you can perform some steps manually / with Imcalc, e.g. subtract the preprocessed images from time point 1 and 2. However, changes could be due to anything, therapy, disease progression, normal developmental changes, ... To infer whether changes are due to therapy you would need some control subjects, in your case patients without the therapeutic intervention plus possibly, also healthy controls. You might want to look at Mühlau et al. (2009, https://dx.doi.org/10.3174/ajnr.A1390 ) for a study in which single patients are contrasted with a group of controls. Their study is not longitudinal, but for that purpose you could just go with e.g. the difference of two GM images & forward these into the tests.
