At Bangor University we are also thinking about how to improve the enquiries referral process. Currently we have a very similar system to the one that Marty has described. We have a separate email for academic support enquiries, as well as students and academics being able to contact individual subject librarians, via email/phone/dropping in to the office. We also have a library chat service, covered by customer service staff.
I was wondering if you had received any further replies from other institutions, off list? If so, I would be very interested in hearing about them. My messages to your work email keep getting bounced back !
