The users are associated with their assessments via the collection of groups they are members of. A common problem with WebPA occurs when a staff member edits their groups in "My Groups" but doesn't update any existing assessments to use the new group structure - each assessment uses a copy of the group structure, to avoid accidental changes once it opens for submissions.
So, a student will belong to a particular group (user_group_member table), and each group will belong to a particular collection (a field in user_group_member). Each assessment record will have a collection associated with it (collection_id field).
If you find the relevant records for students already in the right place you'll be able to make up an appropriate row entry for the missing student and register them in the correct group.
I'm not sure if the LTI connectivity complicates matters at all, but with the above info you should be able to work out any discrepancies.

I have an open assessment and a student in one of the groups. However, the student when logged in shows no assessment. Other students have the assessment link OK. Looking in the database, I can’t see a table showing a relation between user_id and assessment_id. Where is that relationship in the database?
