Thanks for all these reports. I fear the problem with version 2.4.40 is that database relays are not working fine any more. Database relays are only present in top BDII and ARC CEs slapd config files. This is why the crashes only happen in these services. I wasn't able to reproduce the problem either in resource or site BDIIs.
For this reason, I would say it's safe to upgrade all services except those ones. Although I will check with MW Officer to see what he considers we should recommend to do. And in any case we will report this back to openldap team.
