Hi,
REFMAC should never crash like this, no matter how the input MTZ file was created. If it does, it is a bug in the program which is triggered by your data, and should be brought to the attention of the author of the program (Garib Murshudov). He is the one who can fix this. (The same holds for any other program, like XDS.)
As others suggest, sometimes inspection of the data can give a hint to a workaround.
