There are two good options for this:
• IFormBuilder I believe this is the best option. You create your forms on a web browser. Once you create the final form, it gets uploaded to your device (iOS/Android). You only need to be connected when you upload the form. Furthermore, once the device has loaded the form, you don't need to be connected at all to capture data. Then once you are done doing the survey, just go back to the office and the device will automatically upload the data. Once the data is uploaded is stored on the cloud (for free) and you can export to pretty much any format with no limitations.
The best thing about this is the integration with the tablet/smartphone hardware. People can take pictures, annotate pictures, sign on the tablet, etc. Therefore you can have a more rich survey compared to other approaches. Another plus is the integration with Geographic Information Systems in case you need to add spatial analysis. Moreover, the form logic and booleans uses basic JavaScript which makes it extremely approachable.
• Google FormsContrary to other web forms Google allows to extract all the data for free. Nevertheless, check the privacy and ownership issues. Since just as Facebook, you give authorisation to Google to view your data. Which if you are at a university, and you need to get ETHICS approval, that needs to be mentioned. Otherwise you will go through some privacy issues.
