I think you're looking at an older version of the guidance. We spotted this and have since amended it to ask for a "_ALERTS" and "_CREATININES" suffix to the file names.

We'll accept IP, OP and COM. If your system can't distinguish between Outpatient and Community (inc. GP) requests these should be coded as OP. A&E requests should be coded as OP.

> If we are sending NHS number and name, why are we including full address
> as well - surely against caldicott guidelines.

Not all laboratory systems have NHS Numbers so the address is needed to allow them to be traced. The Post Code is also required so we can geographically analyse the patients.
