We attach a bar code to the attached form. When the patient is booked in, with a proper Id, then they can fill the correct patient details, sned it to poct and we amend the patient record on the database and then we have a full audit trail. We don't always get them back and intend to audit non compliance with this. We also list which numbers are left with each department as we also use this for glucose as well as blood gases.

Our staff are told to use the date and time for unknown patients when using POCT analysers. However in A&E this may be a bit more hit and miss. They do have an emergancy patient ID barcode they can use for glucose meters if needed.
