I was just about to post back to the list on this one. Yes, absolutely right. There had been an issue some time ago when populating that variable so it got left blank. It finally occurred to us today that it was a redirect problem and that the server address was missing from the URL below. Popped it back in and now working like a charm.

Looks like the LTI launch is successful, it is just not redirecting you to the home page correctly. Have you defined APP__WWW in the inc_global.php file correctly?
