The tool_consumer_instance_guid launch parameter is required by the
WebPA plugin. This should be sent by Moodle automatically, which
version are you running? Moodle refers to it as the Organization ID in
its "English (en)" interface and defaults to the server's domain name.
This works for me in Moodle 2.5 and 2.6.
If you need to disable this requirement change the value of the
protected field in the lti_consumer database table from 1 to 0.
