Thank you very much for your email and the reference. I haven't had the
chance to look at it in detail so I will not be able to comment on it (but
I'll definitely have a look!)
I have found out that the model I'm looking for is from an Italian group and
is a model of "self-empowerment" (I have attached a picture of the model
from a presentation).

It sounds to me like you are describing Rick Snyder's cognitive theory of
hope which involves goal-directed thinking i.e. setting realistic goals,
developing pathways to these goals and cultivating a sense of
agency/empowerment to achieve those goals/overcome barriers etc.
Sometimes called the Wills and the Ways. Is this what you are looking for?
