The problem comes of only documenting in patient notes is if there is a recall of reagents/strips/cassettes etc. from the manufacturer.
If there isn't a separate list of which patients have been tested on what lot number then its very difficult to have the audit trail.
Where we have manual testing we have log books for recording patient test results with lot numbers etc., these are kept with the IQC log books with the meter/analyser.
Very difficult to police though, you can get a rough idea if people are logging results by the number of stock ordered versus what's in the log book, if they don't marry give or take a few then you know staff are not recording correctly, you can then use this as a risk to the Trust etc.

Hi, could anyone help me. Non of my devices are connected at the moment. As well as writing the result in patient notes, Is there any need to record patient results on a seperate spread sheet next to the device. Some of my sites , do not record any information and some do . Does anyone have veiws on what I should implement.
