Many thanks for all your comments and an especial thanks to Dominic at Brunel who did some further testing. As we had guessed, the 4 minute rule only comes into play when someone else tries to access the same book - so if you have a book that many students have to access you are going to get this issue. We're even seeing this with the multiple user access licences.
