I've added a new section, as per your request.

The life-cycle for an approved VO is now:
a) Formal approval (ops team/PMB).
b) Release of VOMS Server records into Operations Portal (or otherwise).
c) Incorporation in Approved VOs document.
d) On going maintenance in Approved VOs document via operations portal
(or otherwise).
e) Formal deletion (ops team/PMB).
f) Removal from approved list.
f) Addition of removed VO into history table to complete life-cycle.
This process will keep the records straight indefinitely, as long as we
stick firmly to it.
