I agree. If you want examples, then you can get excellent ones from the surveys posted on here so it's really not a big job to do. I also think it's a data protection issue - that you need to tell people how their data will be stored, keep/destroyed.

That simply is not acceptable - you must have a brief and ensure your participants know their ethical rights (right to withdraw, confidentiality etc). You must tell participants that you have ethical approval.
