Could we please all just simmer down here?
Why would people leave a useful list because some people got upset about the
student's mistake? If there's a "brain drain", then we don't have a list,
and we don't have access to each other; albeit more supportive terms would
be preferable.

I certainly agree that, at times, the tone that has been adopted on this
list in cases of error has been rather punitive considering that this is
supposed to be a supportive, learning community of PhD researchers.
Is there a code of conduct that anyone is aware of that could be circulated
for information?
There is learning to be had from every situation - even from someone else's
error which serves as a timely reminder of the ethics we are committed to as
researchers.
It would be a real pity for this list to experience 'brain drain' as result
of the actions/expressions of some especially as I, too, have benefited from
the collective wisdom of researchers on this list.

Supervisors are supposed to be training their students and are ultimately
responsible for what they do, therefore I would see it as wholly appropriate
to contact them and alert them to training needs.

I agree. If you want examples, then you can get excellent ones from the
surveys posted on here so it's really not a big job to do. I also think
it's a data protection issue - that you need to tell people how their data
will be stored, keep/destroyed.

very least.
That simply is not acceptable - you must have a brief and ensure your
participants know their ethical rights (right to withdraw, confidentiality
etc). You must tell participants that you have ethical approval.
