Nancy,
Perhaps it would be more appropriate for Aine to discuss the issue herself rather than others assuming responsibility. As has already been highlighted, a mistake has been made but it doesn't necessarily indicate a review of training. Issues such as this may suppress individuals, which in turn could cause even more mistakes to happen because people could fear posting on this site.
