We are running a couple of augmented reality trials with Layar and
Aurasma at York Museums Trust. Both of these apps have their
limitiations but they allow the visitor to trigger audio and video using
the museum object itself - as opposed to a QR code.
They are, however, far from perfect.

That sounds like a job for QR codes, a QR Code scanner and a website
that holds MP3 files at various urls, linked from the QR codes.
If I've grasped what you're hoping for correctly...
