I am currently preparing just such a survey (with 20 possible branches)
- I found that the service provided by my university did not readily
offer this and so I have set up my own site just for my survey using the
Limesurvey open source software. I think that the hosted service on
limeservice might be worth a look at but there are many other offerings
too. As others have said I think the important thing to check is that
they will support the branching that you need.
