Hi Andrew,
The most likely explanation is your site's permissions to perform the
upload function have been changed. That "permissions issue" is most
lilkely a change made at your hosting provider.
...however, the grab image didn't come through - so that may not be
correct. I'd be happy to have a look at the screenshot if you'd like to
mail me off-list.
