There isn't a template for the cover but you can see examples here: http://www.cilip.org.uk/jobs-careers/qualifications/cilip-qualifications/revalidation/pages/revalidationexamples.aspx. They'll also show how people have used the personal statement. The summary can be up to 500 words but you'll see that it may be less if the rest of the statement has taken up more space.
Many Revalidating members are mentors themselves so it will be interesting to hear how many have mentors. I didn't but am probably in the position where I'm surrounded by people who are great examples of good practice so end up having lots of mentors.

Hi folks,
I wonder if anyone can help me out please? I’ve registered as a reval candidate and have just started to look at putting together a portfolio but I need pointing in the right direction and have a few questions:
What should be included on the front cover? I haven’t seen a template so I’m guessing there isn’t a pre-defined structure to the cover.
The personal statement template includes a heading Training /Development Activity. How does this differ to the activities I’ll include in the CPD log?
Also, do I need to reference every activity in the 500 word summary included in the personal statement template? 500 words is already tight without having to do this!
Do I need to include a piece of evidence for each activity? I’ve heard that the reval portfolio isn’t as large a doc as the chartership portfolio so I’m not really sure how much to include without going over the top.
And lastly – just out of interest – do many reval candidates have a mentor? I'm not sure whether to look for one or not.
Any replies would be gratefully received.
