I will certainly complete your survey, and you may be interested in what has been done previously. I attach surveys from U. East Anglia (2008) and also a US based one, ALA MW 2010 is here: http://epublications.marquette.edu/lib_fac/5/ . (I have anonymised the UEA data).
Both of these have significant contributions from practitioners. I have cited them both on numerous occasions, but unfortunately the East Anglia data has vanished from the record last time I checked. I would encourage you to publish your results in a journal to prevent that happening to your results, and cite the two studies above to keep them in the record.
I'm convinced that the copyright checking service we provide is at the essence of providing green open access: IMHO, green OA is at its most fundamental about authors' rights. This is encoded in CTAs and publishers' policies -- which translates to copyright checking. Technical solutions will always be found, but assisting with the fraught relationship between academic authors and the vast number of publishers and their ever changing policies is a very human process, and a lot to ask of authors on their own. Without assistance with copyright, I believe green open access would die, at least at our institution. The data these surveys provide help argue the case for its retention.
