I'm not quite sure what your problem is.
If you already have the affine transformations (which are stored as ascii 4x4 matrices) then you just run:
convert_xfm -omat <outmat_AtoC> -concat <mat_BtoC> <mat_AtoB>
as it says in the usage message.
So if you have transformation matrix files called, for example:
diff2struct.mat and struct2standard.mat
then you just do:
convert_xfm -omat diff2standard.mat -concat struct2standard.mat diff2struct.mat
and that's it.
