Considering that many employers use a very brief questionnaire along the "3 questions" variety that has been discussed in this forum many times and some employers don't ask any post offer/pre employment/pre placement/baseline/call them what you will type questions at all, I think there is a bit of over-advising going on here. Most of my clients use a simple questionnaire they can self administer as no medical info is required. Only if there are issues or for certain roles is a more detailed questionnaire used which is confidentially sent to me. And no, I do not ask for NI numbers and never have in a self employed capacity or any other. I think this is more about the providers earning potential and their system requiring the NI number as a unique identifier which is a bit of a laugh as my experience is that most people don't know what theirs is. My advice is to be very clear about what you and the employer really need to know in such a process balanced with the need to keep it as simple as possible for the employer. They will thank you for this and probably won't thank you for introducing a load of potentially risk averse bureaucracy.

I have now had the opportunity to review this form. There are a number of issues that render this form incomplete and if you were to use this document as it stands, as a legal document would not hold a great deal of weight or credibility.
The following, gives you some indication of what we mean:
• Absence of employee’s full details are missing i.e. full name, address, date of birth, telephone and all importantly. NI number is missing which should be on all medical records.
• Health declaration should have current and historical information, and details of illnesses that run in the family. This is important for the health professial to assess the overall suitability of such candidates against a particular job role.
• No details of previous employment history
