Thanks again to everyone who responsed to my request for examples of literature search sheets.
As requested here is a summary of some of the responses I received;
1) I provide the following:
1. Search strategy with any assumptions / exclusions 2. Indicate which databases / sites / online resources have been searched.
3. Refs and abstracts in date order, source and identify whether full text is available (either in print or online).
4. Useful guidance / reports from selected web sites 5. Always offer to follow-up and discuss results / re-run search with revised criteria / search terms if required.

I usually save the search as a pdf document and email it to the requester. I don't brand the search at all.
I don't categorise or summarise results, principally because the people I carry out searches for usually want everything relevant and would prefer to make their own decisions/draw their own conclusions.
I always send a copy of my search strategy for information.

I include the following email information;
Dear
Please find attached the results of your search as requested. I hope they are of use.
There may well be some duplicates as I have searched more than one database. The databases I have searched are as follows:
British Nursing Index, Cinahl, Medline, the National Library for Health & the Cochrane Library.
For any references where there is a link to the full text please use your NHS Athens username & password to access. (if you need any help with this please let us know)
If you would like any of the full references from those that do not have links please let us know & we will source them for you.

We do a cover sheet summarising the evidence. We then put the results under headings of evidence type starting with the higher evidence first.
