copy of a typical report with all identifiers removed/obscured?
I'm making a collection..
I still can't understand *why* the quality of A&E reports is so abysmal -
unless the records in the A&E department themselves are equally poor?
Looking at A&E data for QP12, I sometimes think that might well be the cause
of the problems - and if it is, that *would be* a *major* patient safety
risk! ;->

I am in the same PCT as MDGH (CECPCT) and get some A&E letters from them
(most from another hospital though). Information quality is no better at
either trust. If you read the practice praise for the system it is all about
electronic transfer not information.
