CONVERT: Treat TYPE=_REAL -> BITPIX=64 as a special case of data scaling.
Hitherto all the scaled integer output data type used fewer bits than
the floating-point values. For 64-bit integers this breaks down.
Also corrected a couple of related type tests from the input data type
to the 64-bit integer output, when writing BLANK header and informing
FITSIO of its value.

Fix floating point tests
They were failing if you used long doubles (-Dusemorebits) so we now
test floating point comparisons properly using Test::Deep and
Test::Number::Delta.

Fix "attempt to free unreferenced scalar" warnings
These occured when the module croaked with an AST error. There was
a problem with the mortalisation of the elements of the error
stack where I was making elements of the AV mortal instead of just
the AV itself. Now we just store the SV*s from the global stack
directly in the local AV* and just increment the reference counter
whilst also clearin the global stack.
This patch includes a test and a cleanup of the exception triggering
code so that we no longer call back into perl simply to create
a string and croak.
