There is no such thing as radiological or neurological convention for 3D
images. Those are concepts from a time people believed brains were flat.
One speaks of left-or right handed image handedness in 3D, as that fully
defines the coordinate systems that are used. To learn more about

you the possible handedness of 3D coordinate systems.
In SPM-practice: the coordinate system used is for nii data is defined
in the data itself, so it is up to you. For analyze data (that is, spm2
and older) you should set this in the default values (spm_defaults). In
SPM8, you should specify this handedness in your data, and is usually
dealt with the package (outside of SPM?) creating nii data from your raw
scanner data. You should check whether this is done the way you want.
Image handedness will therefore be different from site to site and
scanner to scanner, but most places I visit use right handed
coordinates. Inspect using the DISPLAY button whether the coordinates in
your image behave like a right handed coordinate system or a left handed
coordinate system, and you'll know what your dealing with.
