Self Management Service Health Promotion Worker – Waverly Care

£23,230 pro-rata 18.75 hours per week. Overtime will be reclaimed as time off in lieu.

Location:        Edinburgh

Closing date: 30 Mar 2012

 

The Waverley Care Self Management Service supports individuals living with HIV and /or Hepatitis C to better self manage their respective long term conditions. The Self Management Service provides opportunities for one to one support and group work programmes which are designed to empower individuals provide them with the knowledge, skills and advice they need to make properly informed decisions concerning every important aspect of their life.

The post holder will be responsible for delivery of health promotion interventions, ranging from the facilitation of thematic group work, one to one self management interventions and to facilitate peer support opportunities.

Candidates should have good knowledge of issues surrounding people living with HIV and/or Hepatitis C and experience of community based work.

The post is for one year only commencing April/May 2012, ending March /April 2013
