If you don’t have OH software you need to have an Excel spreadsheet and each column should be a single detail you want to report on. Drop down lists are good to keep data entry consistent, date columns should be always use slashes and not dots (excel things a dot is a decimal point in a number and will treat it like that. learn to use pivotal tables and the graph toll in excel and you will be off and running.
The main thing is
What do you want to know and why - thes will inform the columns of your spreadsheet
will your data be secure (as it will more than likley contain personal infomration)
what does the clinet want to know (and is that a resonalbe thing to collect
will your data enable anyone person to be readily identified (like reporting on the number of woomen with mental health when only one works in the department - I use <5, 5-10, >10 on one report I do on mental health to avoid this problem)
Some Suggested columns
date of referral
no of days to 1st available appointment
no of days employee absent at time of referral
no of days to RTW
outcome of consultation
reason for absence (Mental Health, MSD etc with some breakdon of each if relevant e.g stress, depression , anxiety other MH)
The list could be much longer
