Hi Dave,
We now use a specific health surveillance e-programme for this but previously I used the following headings which you may find useful:-
Background - reason for assessment etc;
Office Layout - diagrams can be useful or a photo;
Heating/Lighting - location of windows, artificial light, glare problems, desk lighting;
Desk - height, space available, under desk space, location of other equipment etc;
Chair - height-appropriate, working order, fabric etc;
DSE - position, angle, ease of use etc;
Other Office Furniture - filing cabinets, photocopier, location of these to individual's workstation etc;

Advice to Manager
This is just a list of headings I used and is not by any means definitive! I'm sure other list members can offer advice on this.
