Actual knowledge of the operations of the organisation or company is
unfashionable among managers these days.
An area where the NHS has not lagged behind developments.
Hence MMC/MTAS and a range of IT failures.
And, one would be unwise to dispute, Stafford.
TO be fair, deliberate criminal activity is not what most regulation
is intended to prevent.
