If the images have been spatially normalised, then you would simply
multiply the vector of voxel indices (numbered from 1..N, rather than
0..N-1) by the voxel-to-world mapping in the image header.
Nii=nifti('wblah.img');
mni_coord = Nii.mat(1:3,:)*[i;j;k;1]
