I don't have any to hand but lots I see now only have 2/3 key questions . You need to sell it to your employer the possibility of opening themselves up litigation and quote the new laws around questionnaires and employment .
They can by all means ask what the choose but is the information asked of benefit and what will they be using it for .???
You should be screening under your medical legal guidance .
