Hi

It sounds like there may be many yellow flags in this case that are
affecting the outcomes. Normally in such cases I refer to the inhouse OT
service as they are excellent at progressing these cases, identifiying
barriers to restoration of function, giving clear and detailed return to
work plans based on job demands and providing clear evidence of functional
capacity.

It might be worth considering, as already mentioned, what the required
outcomes are and who wants them; IMHO I can't see a GP report helping a
return to work from an OH persepective.

  If you feel the need to get more info from the GP then go for it, as
  detailed in the response below it may be useful - and you can ask what
  you like as long as the individual has given relevant consent and knows
  why you are asking the questions.

  In similar cases I would be working with physios to try and expedite a
  return to full function, and for the physio to be outlining to me why
  the rehab programmes havent been successful (do the rehab programmes
  involve treatment or is it a gradual re-introduction to work?)

  But - who has asked you to write to the GP? Could the assumption be that
  this  isnt your preferred course of action but that someone esle is
  directing you to do this?  Is the reason for the letter to gain more
  clinical information or to mitigate any compensation that may arise from
  a personal injury claim?

  Yes, I believe you need to understand this employee's past medical
  history; issues which may impact on a successful return to work. I
  would be interested in what the actual diagnosis is and treatment so
  far - you are not focusing on causation of the back injury but rather
  what is potentially adversely affecting the RTW and the GP's opinion
  on the prognosis.
