Hello Mandy,
He has had an MRI scan and one epidural injection for pain, he
unfortunately has not been able to continue with the other two epidurals as
he suffered a pulmonary embolus after the first.
He initially slipped on a wet floor whilst stepping down off of a platform,
it was thought at first to be muscular, since then he has had an MRI scan ,
there has been no clear diagnosis given by his GP on his Fit note other
than back pain, until now he has self certified his absences . His
previous employment history reveals that he had worked in the road
construction industry for several years prior to his employment here, he
denies any previous back problems. Until now we have supported him with
various adjustments and rehab plans which have thus far been unsuccessful,
HR have been managing him and now that he has gone on long term sick, have
requested that a GP report be sought . As mentioned above he has had one
epidural,and at present his pain is being managed by medication.He has also
submitted a claim against the company for compensation.
There are issues around finances too.
I was not sure if I could enquire about any past medical issues relating to
his Back.

Hi Gina,
Yes, I believe you need to understand this employee's past medical
history; issues which may impact on a successful return to work. I
would be interested in what the actual diagnosis is and treatment so
far - you are not focusing on causation of the back injury but rather
what is potentially adversely affecting the RTW and the GP's opinion
on the prognosis.
