I think the only time I've ever seen this sort of thing it was either a matter of using your own as Peter says or buying really cheap ones from Poundland and then incorporating that cost either yourself or as part of lending costs. So in your case maybe think if the cost is worth it for those people that don't already have access to ear or headphones.

This might not help, because it could be difficult to be sure of, but
a good number of patients would probably have earphones of their own
which came "bundled in" with radios, etc. - personally I have
earphones ... coming out of my ears ;-)
