missed it till now: the King's Fund produced a paper "Avoiding hospital
admissions: What does the research evidence say?" in Dec 2010 which is
available online here:

an interesting summary.

I have been searching for what works to reduce hospital admissions
(non-elective) including frail elderly and different populations eg

Have you undertaken a recent search or summarised the evidence on this
