We've investigated several 'unacceptable use' incidents over the years from several publishers. In all cases our logs for EZproxy, LDAP etc have been able to trace the user, or to narrow it down to a very small number of possibles. This is all standard practice. I think IEEE are just making a rod for their own backs by being so explicit about the steps that would be taken.
Publishers also provide us with their logs for these incidents and sometimes that leads us to defend the actions of the user against accusations of unacceptable use. For example, we told a publisher that we did not think it unreasonable for a user to download several articles from a themed issue; it wasn't the user's fault that the publisher choose to gather all those articles of interest to the reader into the same issue. The publisher didn't argue the point.
Most incidents have just been users innocently downloading lots of articles while they were online so that they could decide later (offline) which ones to read in detail. Curiously, most years we see some big usage stats spikes that haven't triggered the publishers' systems' unacceptable use thresholds...

Nicole is correct. We queried the requirement with IEEE and our access management server logs would suffice. If a publisher detects a suspicious event, something like systematic downloading, the institution responsible would be asked to investigate and take steps to remedy any non-compliant usage. We've never received such a request from IEEE during the eight years or so that we've had online access.
