Segmentation etc were not designed for PET or SPECT, as they don't
deal with the degree of smoothness of the images. Mohamed's
(http://www.fil.ion.ucl.ac.uk/~mseghier/) lesion segmentation tools
may be of help with your MRI data though.
In general, not everything can be easily automated yet (one of my
students is a regular ITK-snap user). In principle though, any
labelling that a human expert can do, a computer should be able to do
eventually.
