I forgot to add that this process is after a job offer has been made and that HR inform the individual that the offer is subject to satisfactory screening whether that be paper screening or health surveillance. Only where the requirements are intrinsic to the role are questions asked prior to job offer. I don't think I would ever deem someone completely unfit for a role. I would always recommend modifications and work with HR to see whether these were reasonable and achievable.
