Notwithstanding that you need to ask this of a properly qualified accountant my understanding is that the VAT is exempt where you are directly advising an employee and then the employer about a health matter following a medical like consultation and that it would apply where there was no direct contact per se and you were advising the employer about matters more generally
So screening by questionnaire for instance is likely to attract a vat liability if you are of course above the vat registration limit to start with.
Again I am not an accountant or lawyer and as ever best to get professional advice
