Dear Martin,
I am not certain that this fits your needs but RunCoCo has developed CoCoCo (community contributed collection) software. This will be released as open source in the new year. We used CoCoCo for the Woruldhord Anglo-Saxon collection this year, and we are still working on it. You may download the version we used for The Great War Archive pilot collection in 2008, see
http://projects.oucs.ox.ac.uk/runcoco/resources/cococo.html
It's based on Ruby on Rails. The version we are working on may be adapted for any collection purpose. Attachments / file uploads are not compulsory - a contributor might enter text only. Fields and forms can be adapted when you set up the software on your server.
Best wishes, Ally

Subject: Form that allows file upload
Hi all
I am looking for a simple way for content contributors (mainly people in smallish museums) to submit some info through a set of short text fields, and also to upload some assets (images, videos, worksheets,
etc).
Google forms seemed like a possibility, and the forums say they have been looking into it (for some time...) but they don't seem to offer it
yet.
SurveyMonkey specifically exclude the possibility of appending files to a survey.
I could use WordPress, but that would mean having to create an account for each person who will use it, which is a bit clunky - though
possible.
Posterous, Tumblr could work, but posting via email is the only way for people without an account to post, which wouldn't be good for large attachments.
DropBox etc?
I guess the idea system is a kind of cross between an online form and a yousendit.com type of service.
In case this all seems a bit Heath Robinson, this is a tryout for a system that will probably be built into a CMS for scaling up.
Does anyone have any other ideas?
Martin
