Hi Peter,
It sounds like your database was not updated along with the new Web-PA code. There are a couple of new fields in the assessment table, and without them assessments cannot be created properly.
There's info on a patch for this here:

but basically, your tech guys need to run the following SQL commands on the Web-PA database - they should change the table-names as appropriate, depending on what your install is called.

There seems to be a few more problems which I have encountered with
v1.0.0.6 of webpa. The first one is with creating the assessments. I
have tried this a number of times. Each time I am taken to a series of
pages (1) selecting the assessment time and date (2) choosing the
assessment form (3) selecting my collection (4) selecting assessment
type (I select the self and peer assessment option (5) The summary of
the created assessment and I am then asked to select "finish". I then
get (6) which says that "Your assessment has been created". However,
when I now select my assessments. There are no assessments listed. Any
idea what might have happened here? Could it be that we have forgotten
to install a program dependency, etc. I noticed that clicking emailing
students notifying them of a new assessment does work. But either the
assessment has not been created or it is just not visible.
Anyone have any ideas about this one. Could it be a bug in the v1.0.0.6?
The other problem has to do with assignment of passwords. I have
imported user data with usernames and passwords. Students are unable to
log in. The only other way is for them to request a new password with
"Forgotten your password?". This sends an email to the user. with the
request to click or copy the link "accounts/......." instead of the full
address of "http://webpa.food.leeds.ac.uk/accounts/......." If the user
then replaces accounts with the suggested linked, then it works OK and
the user is now able to log into webpa.
Any suggestions to solve the first problem would be great.
