Dimitris, for the UK, this information is a bit spread around. You will find some references to it in the following guidance document on transport infrastructure http://www.dft.gov.uk/pgr/roads/tpm/ltnotes/ltn208.pdf. See section 10.2 in particular. You will also find reference to pedestrian and cyclist signals on page 7 of the following Traffic Advisory Leaflet http://www.dft.gov.uk/pgr/roads/tpm/tal/signsandsignals/tcbls/ralprinciplesoftrafficco4105.pdf. I don't know if there are other references in some of the other Traffic Advisory Leaflets on this page: http://www.dft.gov.uk/pgr/roads/tpm/tal/.
You should also try to find some guidance from the Netherlands. They have some quite different practices, including in some cities "cyclist only" phases at cross roads, where all other traffic has to stop and cyclists are allowed to move in all directions. Practice in Denmark would also be of interest - the system there is quite different to anywhere else I have travelled. In most circumstances cyclists turning left are required by law to raise one hand, stop on the right hand side, wait for an opening and then cross both carriageways at the same time.
