I found Sayeed Khan's recent lecture very helpful when he highlighted the difference between health screening and health surveillance (i.e. that screening is about identifying health issues that may impact work and surveillance is identifying any health effects from the work they are doing). So your HR colleagues may want to think a bit more about what they are wanting from this.
The health surveillance stuff should be risk based and it looks like you are already covering that. Health screening might include musculoskeletal conditions as well as general health (mental and physical).
Have you talked to them about what the company will/won't do as a result of anything you find out from this screening. Are they willing to support flexible working, adjustments etc? Will they look at ergonomics if that is raised as an issue? Will they invest in health promotion activities?
