May I suggest the protocol as mentioned in the UK workplace guidelines.
These guidelines can be found on the LTG website, the link is:

then scroll down for the guidelines, I think it's the fifth document down.
The protocol is in appendix 1.

This is not something I now deal with but in my previous place of work
the individual had to produce a urine sample in the OH department. It
was not acceptable for him/her to bring one in as it could belong to
anyone!!! We ensured the toilet was "out of bounds" to other users and
we put food colouring in the cistern and toilet bowl for obvious
reasons! The wash basin was outside the toilet cubicle and a member of
staff escorted the client to the toilet and waited for the client by the
washbasin. This ensured that there could be no "fiddling" going on. The
client was asked to remove any jacket (in case a urine pot with a
friend's urine inside was hidden in a pocket!) and there was always an
expectation that the urine specimen would feel warm as it was fresh....
Frank, would you be able to write a procedure based on this information?

It is not just the form but the procedure behind it. People walking in
with a full urine pot is not acceptable and I need to push this real
hard hence I need a robust set of procedures before I get kicked off
site by the client.
