You should include both controls and patients in the template. If you
have equal numbers of controls and patients, including them all is
probably the best approach. If you have significantly different
numbers of controls and patients, you will probably want to create a
template from a subset so that there is equal representation of
controls and patients; this ensures your template (and thus
normalization) won't be biased towards the larger group.
